Note: Both PDF and Word versions of the form are provided here. If preferred, the Word version can be completed as a softcopy and then printed or saved and e-mailed.

Option 1

Discounts
There are two discounts that may be applied against program fees.
Referral Discount
A space is provided on the Registration Form for new families to indicate the name of the family who referred them to Cricket Academy. The family named will receive a $15 discount on their program fees. This discount is not redeemable for cash and is not transferrable.
Multiple Family Member Discount
Families enrolling more than two family members may deduct $15 for each subsequent family member (e.g. third, fourth, fifth family member) when tabulating their total. This discount applies to the number of family members only, not number of classes. See an example below:
